Preparing for Your Visit

To streamline the registration process and minimize your personal expenses, it is important to take the following steps before your appointment.

Step 1: Find out if your policy or plan requires a preauthorization or referral. If so, contact your insurance company and find out what you need to do to obtain one. We will assist the referring physician with the process of obtaining the authorization for your exam.

Step 2: Come to your appointment with your insurance card, name and address of your plan or insurer(s), name and address of your claims payer (if different from insurer), your ID and group numbers, and the name of the policyholder or subscriber. If applicable, for workers' compensation, your approved C-9 is required.
