Schedule An Appointment

Making an appointment at California Imaging & Diagnostics is simple and easy. You or your doctor can schedule your appointment. You will need a referral form or requisition form from your doctor, and you may need pre-authorization from your insurance company or HMO.

The following information is necessary to schedule a radiology exam:

  • Patient’s full name
  • Date of Birth
  • Exam being ordered
  • Reason for exam
  • Is Pre Auth required?
  • Insurance name and policy numbers
  • Ordering physicians name

INSURANCE

California Imaging & Diagnostics is an approved provider for most major insurance carriers and health plans. We will submit your claim, and any secondary claims, to any insurance company under the benefit coverage you specify, including workers’ compensation, auto insurance, and personal injury.

Ultimately, the patient is responsible for payment of the account, including any balance not covered by insurance. We will send you a statement listing payment made by your insurance company as well as the balance you may owe. Please contact your insurance company if they have not paid for your procedure after 30 days.

Preparing For Your Visit

To streamline the registration process and minimize your personal expenses, it is important to take the following steps before your appointment.

Step 1: Find out if your policy or plan requires a pre-authorization or referral. If so, contact your insurance company and find out what you need to do to obtain one. We will assist the referring physician with the process of obtaining the authorization for your exam.

Step 2: Come to your appointment with your insurance card, name and address of your plan or insurer(s), name and address of your claims payer (if different from insurer), your ID and group numbers, and the name of the policyholder or subscriber. If applicable, for workers’ compensation, your approved C-9 is required.

MEDICAL RECORDS AND IMAGES

California Imaging and Diagnostics’ patient portal allows patients to access and send their reports and imaging. Please review the guide below to access your images and reports through the portal. California Imaging and Diagnostics needs your email and mobile phone to authenticate your identity and permit access to your imaging and reports. Please call (866) 419-3372 and select option 2 if you need to update your Patient Record with your current email and mobile phone number.

ACCESSING THE PATIENT PORTAL:

  1. Click here
  2. To log-in, enter: First Name, Last Name, Date of Birth (MM/DD/YYYY)
  3. Read and scroll through the terms and conditions, and check the box to select “Submit”
  4. Select your email address or mobile phone number to receive a security pin, and select “Submit”
  5. Enter the pin and select “Submit”
