Making an appointment at California Imaging & Diagnostics is simple and easy. You or your doctor can schedule your appointment. Either way, you will need a referral form or requisition form from your doctor, and you may need pre-authorization from your insurance company or HMO.

- Exam being ordered
- Reason for exam
- Is Pre Auth required?
- Insurance name and policy numbers
- Ordering physicians name
